Well, if you need modern features, you need to go to http://www.qmail.org and apply patches from that page. There are tons of them.

i used dave sill's "The Qmail Handbook"detailed step by step installation, but it always depends what os or which version you use.
go for http://www.ornl.gov/cts/archives/mailing-lists/qmail/ for tons of information, questions and answers
my suggestion is to stick to 1.03 which is original version. any other version will surely complicate the management.
