The city hall where your gf is resident there. Take your passport with you, that's it. 80 for registration, and 20 for clerk certificate.

No, normally they don't ask for it. Basically if you say "I am single.", they will consider your words as an oath if they could not find you in their DB. Actually your single certificate is an oath in written.
